EPF Form 10D is the official Employees’ Pension Scheme (EPS) application used to claim your monthly pension after retirement or in special situations like disability or death (for dependents).
Who Can File Form 10D?
- You, the EPF member, on retirement at age 58 or early retirement (between 50–58)
- Eligible dependents: widow(er), orphans, guardian, nominee, or dependent parents in case of member’s death
Documents You Need
- Filled Form 10D (2 copies if needed in different regions)
- Proof of identity & UAN (Aadhaar linked)
- Pension Payment Order (PPO), if already available
- Specimen signatures/thumb impressions (2 copies)
- Three passport-sized photographs
- Bank details: account number, IFSC, cancelled cheque/passbook
- Scheme certificate or previous EPS certificate details
- Additional medical certificate for disability pension
- For death claims: death certificate and family member details
How to Fill Form 10D (Offline)
- Claimant section – Indicate who’s claiming (member, widow, guardian, etc.)
- Type of pension – Choose superannuation (58+), reduced (50–58), disability, widow & children, orphan, nominee, or dependent parent
- Member details – Name, gender, marital status, DOB, father/husband’s name
- EPF account info – UAN, scheme certificate/PPO number, region & office code
- Employment info – Employer name & address, last working date, reason for leaving
- Address and communication – Ensure bank and address are in the same state
- Pension options – Tick for commutation of 1/3 pension or full withdrawal
- Nominee/family details – Provide spouse, children, or dependent family info
- Bank account section – Fill bank name, branch, account details; attach cancelled cheque/passbook
- Scheme certificate info – If held, provide control number and issuing RO/SRO
- Documents list and signatures – Member signs form, employer fills employment details and signs/seals
How to Submit Form 10D (Online)
- Login to EPFO UAN Member Portal → Online Services → Claim (Form 31, 19, 10C & 10D)
- Select “Monthly Pension (Form 10D)”
- Verify your bank account; upload cheque image & Aadhaar authentication via OTP
- Fill details online, tick declarations, upload required documents, enter OTP and submit
Note: Employer verification is automatic via UAN portal credentials.
Processing Time
EPFO typically processes Form 10D claims within 30 days of submission.
